-3-
2.03,4 Identification of Interest (2.03.4)
If there have been changes in the owners or leaseholders of the surface or
underground coal rights this section of the application should be changed to
reflect this.
I. General Compliance
1. An up to date mine plan map should be submitted to replace Map TP-2,
This map should show the present extent of mine workings.
2. The list of Revisions in the beginning of the permit application was last
updated October 21, 1983. A revised page reflecting the current status
of the revisions should be submitted. Also all revisions and stipulation
responses should be inserted into the permit application.
3.' Section 7.03.8 Permit Term Information, a current proof of Insurance
should be submitted to be inserted into the permit application.
4. An approved Air Emission Permit should replace the Air Emission
application form that is currently in the permit application.
5. The expired County Special Use Permit in the application should be
replaced by a current permit.
Reclamation Plan
1. In the original Findings, the Division noted that the reclamation plan
was quite generalized, and therefore included several stipulations to the
permit which required additional information. These requirements
included revegetation standards, more detailed backfilling and grading
estimates, and more detailed operation plans for material movement.
The operator should modify the reclamation plan to incorporate the
requirements of these stipulations. The plan should include alt
assumptions as to where material is being moved, and the volumes of all a
backfilling and grading operations. The narrative should also explain
how the mine bench will be graded. Apost-mining topography map should
also be submitted, and the post-mining topography should be included in
the mine site cross-sections.
2. The lower (Lewis Gulch) pond will be removed as part of the initiai site
regrading. The operator should therefore include some alternative method
of sediment control for the areas rooted to this pond (i. e. berms. straw
holes etc.).
- 3 -
