-3- <br />2.03,4 Identification of Interest (2.03.4) <br />If there have been changes in the owners or leaseholders of the surface or <br />underground coal rights this section of the application should be changed to <br />reflect this. <br />I. General Compliance <br />1. An up to date mine plan map should be submitted to replace Map TP-2, <br />This map should show the present extent of mine workings. <br />2. The list of Revisions in the beginning of the permit application was last <br />updated October 21, 1983. A revised page reflecting the current status <br />of the revisions should be submitted. Also all revisions and stipulation <br />responses should be inserted into the permit application. <br />3.' Section 7.03.8 Permit Term Information, a current proof of Insurance <br />should be submitted to be inserted into the permit application. <br />4. An approved Air Emission Permit should replace the Air Emission <br />application form that is currently in the permit application. <br />5. The expired County Special Use Permit in the application should be <br />replaced by a current permit. <br />Reclamation Plan <br />1. In the original Findings, the Division noted that the reclamation plan <br />was quite generalized, and therefore included several stipulations to the <br />permit which required additional information. These requirements <br />included revegetation standards, more detailed backfilling and grading <br />estimates, and more detailed operation plans for material movement. <br />The operator should modify the reclamation plan to incorporate the <br />requirements of these stipulations. The plan should include alt <br />assumptions as to where material is being moved, and the volumes of all a <br />backfilling and grading operations. The narrative should also explain <br />how the mine bench will be graded. Apost-mining topography map should <br />also be submitted, and the post-mining topography should be included in <br />the mine site cross-sections. <br />2. The lower (Lewis Gulch) pond will be removed as part of the initiai site <br />regrading. The operator should therefore include some alternative method <br />of sediment control for the areas rooted to this pond (i. e. berms. straw <br />holes etc.). <br />- 3 - <br />
