December 18, 2001 Insaection by Bob Oswald: <br />No Permit Identification Sign or Permit Boundary Markers: <br />The lack of a Permit identification sign and Permit boundary markers is noted as a <br />"problem" and a Corrective Action was specified. <br />Response: <br />In January 2002 the Operator installed a correct Permit identification sign and <br />provided photographic evidence. <br />The Operator is submitting a new Proposed Affected Area boundary with this current <br />information package. Upon reaching concurrence with the Division on the Proposed Affected <br />Area boundary, the corners of the area will be marked in the field. <br />Inadequate Site Map: <br />Following the June 2, 1998 letter from the Division, which noted a number of items <br />that needed to be revised and updated on the site map, the Operator failed to provide a current <br />and accurate site map. As a result of the Operator's failure to respond to the Division's <br />inquiries, the lack of a current and accurate map is classified as a "problem" and a Corrective <br />Action was specified. <br />Response: <br />The Operator is submitting herewith an updated and correct site map. <br />Specific Site Features: <br />Numerous items are not shown on the site map which was submitted in April 1998: <br />trash pit on the dump, small ore stockpile, power supply incorrectly located, shop building <br />size does not match map, topsoil storage area and future expansion areas do not exist as <br />shown, dump boundary location is in question, etc. <br />Response: <br />The updated and correct site map submitted herewith addresses the questions posed in <br />the December 18, 2001 site inspection (the matter of marking boundary corners remains to be <br />resolved). The trash pit has been removed and additional grading has been conducted on the <br />dump surface. Recent site maintenance activities (November 2002) are not shown on the <br />enclosed map, as this work was done after the site was surveyed. Survey updates will be <br />made to incorporate the minor site changes that have been made, and a revised site map will <br />be submitted to the Division with the next Annual Report for the St. Jude Mine. <br />February 4, 2002 Letter from Division: <br />Permit Identification Sign: <br />The Division acknowledges the installation of a Permit identification sign at the St. <br />Jude Mine. The permitted area boundaries have not been marked, so the Corrective Action <br />item is still open. <br />Response: <br />As noted in the preceding comments, the Operator is ready to mark Affected Area <br />boundaries upon reaching concurrence with the Division on the Proposed Affected Area <br />boundary submitted herewith. <br />
