Phase Nc Summary No. 16 Page 2 043-2170
eastern portion of the VLF. SLF material was tested, conditioned, reworked, retested, and maintained
prior to SLF surface acceptance.
Ames continued excavation of the Phase IVc anchor trench on the eastern section of the Phase IVc
VLF. Ames excavated approximately 100 feet of anchor trench in preparation for geomembrane
installation during this reporting period.
Ames continued production of bentonite-amended SLF at the screen plant and pug mill located above
the Ames materials yazd. Ames also continued production of Drain Cover Fill (DCF) material at the
screen plant by the secondary crusher.
Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase
IVc OSA. Geomembrane was installed in the Phase IVc OSA on the south facing slope of Raven Hill
downward to the Phase IVb/IVc tie-in. Poly-Flex deployed 80-mil single-sided textured LLDPE
geomembrane over an azea of approximately 102,274 square feet in the Phase IVc OSA during this
reporting period. Geomembrane panels C-706 through C-735 were deployed, seamed, and tested by -
Poly-Flex. Destructive fusion and extrusion seam samples DFc-252 through DFc-262 and DXc-26
through DXc-27 were mazked and removed. Destructive fusion and extrusion seam peel and sheaz
testing on samples DFc-236 through DFc-262, and DXc-26 through DXc-27 were performed and
passed per project specification.
Ames continued hauling, placement and grading of DCF material into the Phase IVc OSA in the central
and eastem sections of the VLF, Raven Hill azea. Ames placed DCF material and High Volume
Solution Collection (HVSC) piping per the project plans onto approved geomembrane panels.
A map of the area worked during this reporting period is provided in Attachment C.
Field Monitoring and Testing Activities
Golder observed construction activities associated with Phase IVc earthworks. All constmction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications.
Golder observed the excavation and construction of the LDT in the Phase IVc VLF for trench
dimensions (1' wide by 1' deep), slope, and materials used.
Golder observed excavation of the Phase IVc anchor trench for proper location, dimension (2' wide by
3' deep) and maintenance prior to geomembrane deployment. Anchor trench excavation met or
exceeded the requirements of the specification.
Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVc OSA for
subgrade preparation, thickness, compaction, moisture-content, and materials used. The Golder CQA
Earthworks Monitor performed seventeen nucleaz moisture-density tests (SLNc-202 through SLNc-
218) and performed five nucleaz moisture-density retests on SLF that required reworking (SLNc-212a,
SLNc-213a, SLNc-214a, SLNc-217x, and SLNc-218a). Field test results are presented in Attachment
B.
rnaz~rv~i5a~uzz-w ~za znoa Golder Associates lnc.
