<br />Mid-Term Review 2 November 6, 1985 <br />a. A revised table of contents must be submitted for the permit <br />application. The table of contents must include all additional <br />or revised exhibits and maps, and a list of all supplemental <br />material that has been submitted, including stipulation <br />responses. This information is necessary to insure that the <br />permit application is accurate and complete. Also, any material <br />not currently in the application must be inserted in the <br />appropriate locations. <br />b. Because of the changes in management at the mine the permit <br />application form found in the front of Volume I must be revised <br />to show current names, address, contacts, etc. <br />c. Section 2.03.5 - compliance Information must be updated in <br />accordance with Rule 2.03.5(3), including a statement regarding <br />each violation notice received since the application was filed <br />which describes the following: <br />(a) The date of issuance and identity of the issuing regulatory <br />authority, department or agency; <br />(b) A brief description of the particular violation alleged in <br />the notice; <br />(c) The date, location and type of any administrative or <br />judicial proceedings initiated concerning the violation, <br />including, but not limited to, proceedings initiated by the <br />applicant to obtain administrative or judicial review of the <br />violations; <br />(d) The current status of the proceedings and of the violation <br />notice; <br />(e) The actions, if any, taken by the applicant to abate the <br />violation; and <br />(f) The final resolution of any notice of violation. <br />Soils - Rules 2.04.9, 2.05.3(5), 2.05.4(2)(d) and 4.06 <br />On pages 186-181 of the application the operator made a demonstration, using <br />estimated stripping depths, that there would be enough topsoil available to <br />cover the disturbed areas with 6 (six) inches of topsoil. <br />Since the topsoil has now been stripped and stockpiled, the stockpiles should <br />be surveyed to determine if as much material has been salvaged as the initial <br />calculations had shown to be available. <br />Backfilling and Grading - Rues 2.05.3(a)(b), 2.05.4(2)(a), 2.05.4 2 c , <br />4.09, 4.13 and 4.14. <br />
