'91 Unpermitted water discharge down Cash Gulch into Summerville reported to <br />Division by (Summerville) residents. <br />26 Nov. '91 DMG cites four possible problems and two possible violations, including <br />unpermitted water discharge, rip in pond liner, and sulfide-bearing concentrates <br />in contact with ground near mill. <br />13 Feb. '92 DMG inspection shows previous problems and violations not adequately <br />addressed. Found additional tailings spill on ground next to mill aed tailings <br />impoundment. Required to submit a detailed plan for clean-up of spills and a <br />detailed plan for future containment of tailings, including back-up measures to be <br />taken in line breaks, tank overflows and other spills. (This was never <br />accomplished.) <br />3 Apr. '92 DMG sends NOPV letters to Co-operators for uncoataiaed tailings spills and <br />rips in impoundment liner. <br />l6 Apr. '92 DMG inspection notes (under "Problems/Possible Violations and Corrective <br />Actions") "Uncontained tailings material and erosion channels outside mill <br />building anti tailings impoundment as well as off site between the tailings <br />impoundment and the Hazel A atilt." <br />28 May '92 Board issues NOV's on 3 April issues. <br />15 June '94 DMG inspection notes that "The tailings are level with the lower pond <br />embankment on the NE side of the tailings impoundment. Must prevent <br />overtopping." <br />27 June '94 Colo. MLRD sends letter to W. Tatman discussing importation of ore to the <br />Gold Hill Mill, requiring mineralogic analysis of ore prior to movement of ore <br />to millsite, and proof of ownership of ore by COM, Inc. <br />5 July '94 Boulder County sends letter to W. Tatman stating, "Bringing ore from other <br />mining sites must discontinue. This means any quantity for any purpose." <br />"Storage of tires, automotive parts, and unlicensed and inoperable vehicles is not <br />permitted. Please remove these items within 30 days." "Storage of a camper <br />trailer is not allowed." <br />
