topsoil slippage in one location on the outslope of Bench 3 in 1995, the first year following topsoiling. The <br />operator has taken appropriate remedial measures. <br />Adequate soil cover thickness (18" subsoil/topsoil combined) on the lower benches was documented in <br />the DMG inspection report of June 29, 1994. Adequate soil cover thickness on the upper benches (top of <br />pile and side slopes) was documented in DMG inspection report of January 31, 2003. <br />Several relatively minor maintenance items noted in the inspection report dated April 15, 2003 had been <br />properly repaired prior to the bond release inspection. Based on observations made during the bond <br />release inspection, observations and measurements documented in prior inspection reports referenced <br />here, and information included in the bond release application, the Division concludes that CRDA-1 has <br />been constructed and regraded in conformance with the approved design. The short road segment that <br />has not yet been reclaimed, and which will be used for placement of a relatively small volume of <br />additional coal waste, was properly excluded from the bond release request. <br />Cameo Refuse Disposal Area 2 (CRDA-2) <br />Refuse Area CRDA-2 has been fully reclaimed. The lower three benches of the pile were permanently <br />reclaimed in 1994. The upper lwo benches were final graded, cover-soiled, seeded and mulched in April <br />2002. The final reclamation project in spring 2002 included the construction of a new permanent upper <br />diversion ditch, and riprap installation along the upper segment of the East Perimeter Ditch. Final <br />reclamation included extensive roughening to enhance vegetation establishment, and rock placement for <br />small animal habitat. In November 2002, the old upper diversion ditch located on the steep canyon <br />sideslope above the pile was regraded and revegetated, as was the temporary access road up the face of <br />the pile. <br />Pertinent mapping, cross-sections, compaction data, construction certification, and permanent diversion <br />certifications are included in the bond release application. <br />DMG documented compliance of the lower reclaimed benches and perimeter ditches during a special <br />focus oversight inspection conducted with OSM in August 1997. Extensive slope and ditch <br />measurements were taken, and results were described in the DMG inspection report of August 29, 1997 <br />and report addendum of October 17, 1997. <br />Adequate soil cover thickness (24" subsoilltopsoil combined) on the lower benches was <br />documented in DMG inspection reports of June 29, 1994 and June 8, 1995. Adequate soil cover <br />thickness on the upper benches (top of pile and side slopes) was documented in DMG inspection <br />report of January 31, 2003. <br />Construction certification for the new upper diversion ditch is included in the 3rd quarter 2002 refuse area <br />P.E. certification by Jim Stover. Construction certification of the newly constructed segment of the east <br />perimeter ditch is included in the same refuse certification report. DMG measurements of the new <br />perimeter ditch segment are documented in the Ditch and Channel Measurements section of the <br />inspection report dated April 15, 2003. DMG measurements of the new upper diversion are documented <br />in the inspection report dated May 5, 2003. <br />General configuration of the final graded upper bench slopes and top of pile appeared to be in <br />accordance with the approved design. The top of pile is mildly crowned, with gentle slope toward either <br />the upland diversion or the upper most terrace drain. Bench outslopes are generally in compliance with <br />design gradient and bench heights. DMG slope measurements demonstrating compliance are <br />documented in the Refuse Area Slope and Terrace Drain Measurements section of the April 15, 2003 <br />inspection report. <br />Construction design for CRDA-2 did not include installation of an underdrain. <br />Phase I Bond Release 16 August 15, 2003 <br />
