REPORT ON SITE CONDITIONS <br />CARNATION MINE, PERMIT M-1977-416 <br />March 2003 <br />The Carnation Mine is one of five permitted mines held by the Operator in the Big <br />Gypsum Valley of San Miguel County, Colorado. The Carnation Mine permit includes the <br />mine portal, mine yard, support buildings, electric power service, waste dumps, and related <br />access roads. There are no vent shafts or other outlying feature included under the Camation <br />Mine 110 Permit. <br />Site Features. <br />The enclosed map, CARNATION MINE PERMIT M-1977-416, 1" = 100', shows the <br />features at the Carnation mine site. All features in the Permit area have been re-surveyed, and <br />a number of differences exist between this new map and the existing map on file with the <br />Division. The disposition of various tanks, toilets, ore piles, etc. shown on the old map <br />cannot be reported, as these were removed by previous operators. The current map shows <br />features which have been added since the old map was submitted; added features include <br />waste dumps, catchment ponds, diversion structures, runoff catchment berms, etc. <br />Roads. <br />Primary access into and through the site is via an existing road which traverses <br />through the mine site and continues south of the site. Internal roads within the mine site are <br />also shown on the map. The road through the Permit area is in good condition; minor erosion <br />and cutting does occur on the steep sections above the Carnation Mine. In November 2002 <br />the Operator made repairs and performed maintenance work on the road above the Carnation <br />site. <br />Buildings and Structures. <br />Mine facilities include the Carnation portal, one metal office/shop/dry building near <br />the portal, a storage building near the north edge of the Permit area, an ore loadout structure <br />about 100' north of the portal mouth, and two power lines and a transformer station which <br />provide site power as well as power to other installations in the area. <br />The two existing buildings are in generally fair condition. The office/shop/dry <br />building near the portal covers an area approximately 32' x 28' (building is "T" shaped). The <br />northern building is 32' x 24'. On the north side of the north building a 14' x 16' concrete <br />pad. This pad no longer serves any use, and it will be removed in final reclamation. <br />The Carnation portal was in use during the period of November 1997 through June <br />1999. The Camation Mine has remained on "standby" since that time. In November 2002, <br />the Operator piled dirt in front of the portal to prevent access into the portal and mine. <br />The cribbed ore loadout structure has weakened over time and created a possible <br />hazard. The Operator had previously fenced the structure to preclude accidental access. In <br />November 2002 the Operator elected to completely remove the ore loadout structure, as it had <br />deteriorated further and was no longer serviceable. The 1' = 100" map does not show the site <br />after the ore loadout structure was removed, as this occurred after the site was surveyed. An <br />updated map, showing the site configuration after the removal of the structure will be <br />provided with the next Annual report for the Camation Mine. <br />1 <br />
