This permit amends and <br />replaces SUP #PP2001-046; the amendment included, a 10-year extension, life <br />of permit for 10 years, and a new air inlet shaft, access road and construction <br />.. pad: _ .. _ <br />Location: Mine portal, offices, and rail loadout facilities are located west of intersection of <br /> County Roads 27 and 33; other facilities located throughout the permit area. <br />Legal Description: All or a part of Sections 7-9 T4N, R86W; Sections 32&33 T6N. R86W; Sections <br /> 23-27, & 34-36 T5N R87W; and all of T5N R86W with the exception of Sections <br /> 34, 35, and 36. <br />Period of Permit:. 10 years <br />Board Approval Date: 08/24/2004 <br />Expiration: 08/24/2014 <br />Conditions of Approval: <br />1: .The operator shall comply with all applicable conditions of Section 8.3 (under General <br />Requirements checklist) and Section 8.3.1.4 (Coal Mining Requirements). <br />2. The SUP is limited to uses and facilities presented in the project plan. Any additional uses or <br />facilities, or expansion of operations must be applied for in a new or amended application. <br />3, Any complaints or concerns that may arise from this operation may be cause for review of the <br />SUP"at any time;. and amendment or addition of conditions or revocation of the permit, if <br />necessary. <br />4. The operator shall prevent the spread of weeds to surrounding lands.and comply with the <br />Colorado Noxious Weed Act and Routt County noxious weed management plan. <br />5. All road. and bridge repair and maintenance agreements made with Routt County prior to permit <br />issuance shall be adhered to, and the permittee shall follow the terms and conditions set forth in <br />the Road iVionitoring and Maintenance Plan. <br />Page 1 of 4 <br />
