What other drill holes or water or gas wells exist about the permit area and how have they or will they be permanently sealed and plugged in accordance with Rule 4.07. 6. Information for the current surface water monitoring plan is limited. Please provide a list and an updated map showing locations of streams, springs or seeps, sediment ponds, or other discharge points within the surface ownership lease boundary and adjacent areas which are included in the water monitoring plan. Also, list the required water monitoring parameters for each monitoring site. The current NP DES Permit, No. CO-0000132, lists ten separate discharge points (001-010) with discharge points 003-007 and 009-010 referenced to sediment ponds. The Somerset Mine permit Application received on September 27, 1982 by MLRD indicates a total of thirteen sediment ponds have been constructed in the Elk Creek area. Please identify which of the thirteen original sediment ponds are included in the current permit and list their NPDES discharge point identification number. Clarify why water monitoring is no longer needed for the remaining sediment ponds. Part II. Stipulations and Revisions Required The following is a list of the status of stipulations to the permit. Public Liability Insurance at 0.08 percent of total. 2. Workman's Compensation at 6.92 percent of payroll. 3. Contractors Performance Bond at 2.0 percent of total. 4. Job Superintendent total hours x E20/hr. 5. Engineering and bid specifications. Preparation at 5 percent. 6. Contractors Profit 10 percent of total.
