Phase IVc Summary No. 21 Page 2 043-2170 <br />Field Monitorine and Testine Activities <br />Golder observed construction activities associated with Phase PJc earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers are greater than 50 feet apart) observed DCF placement and <br />verified minirnum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillar <br />Mode1740 articulated trucks). <br />Golder observed placement of HVSC piping in the Phase IVc OSA, verifying installation and layout <br />per the project plans. <br />Laboratory Tests <br />Golder completed QA and QC testing for the remainder of the Phase PJc OSA construction. <br />Meetines <br />CC&V held a Weekly Contractors' meeting on September 28, 2004. Items discussed included <br />Safety, Env~ronrnental, Schedule, QC documentation, and CQA.
