observed pressure change of more than 10% shall be deemed a <br />MIT failure. The MIT shall be performed by pressurizing the <br />casing above the bridge plug set in the well during temporary <br />suspension. At the conclusion of any such test, the applied <br />pressure shall be bled to zero. <br />If a well does not successfully pass an MIT' ornon-endangerment <br />demonstration and the pennittee desires to repair the well, it may <br />do so by submitting an approvable corrective action plan to the <br />Director within 30 days of the MIT failure. Upon completion of <br />the repairs, the permittee shall retest the well and demonstrate <br />mechanical integrity. <br />If a well cannot successfully pass anon-endangerment <br />demonstration (i.e. the above-referenced MTT during the temporary <br />suspension period) and a corrective action plan has not been <br />submitted to EPA for approval, it shall be plugged and abandoned <br />in accordance with anEPA-approved plugging and abandonment <br />plan within 60 days, unless the Director approves of an alternative <br />method to demonstrate non-endangerment and the well can meet <br />those non-endangerment criteria. <br />+ Volumetric flow meters shall be installed on the well injection <br />flow line or, for multiple wells that aze to be temporarily <br />suspended, at a point on the injection manifold flow line that is <br />representative of the common operation of the suspended wells. <br />All meters shall be recorded twice monthly in accordance with 40 <br />CFR 146.33. <br />+ Pressure recording devices shall be installed on the well injection <br />flow line or, for multiple wells that are to be temporarily <br />suspended, at a point on the injection manifold flow line that is <br />representative of the common operation of the suspended wells. <br />All meters shall be recorded twice monthly in accordance with 40 <br />CFR 146.33. <br />+ Monitoring of water quality and water level: The permittee will <br />carry out the applicable monitoring requirements identified in the <br />Surface Water Monitoring Plan (Appendix I). The pernrittee shall <br />utilize the applicable analytical methods described in Table 1 of <br />0.40 CFR 136.3, or in Appendix III of 40 CFR 261, and the <br />QA/QC methods approved in the Appendix I Water Monitoring <br />Plan. <br />Draft Modification No. 9 UIC Area Permit No. CO30858-00000 Page 16 of 44 <br />
