monthly for each well in service and reported quarterly. <br />A subsidence report shall be submitted to the EPA annually. The report <br />will contain data from surface monument measurements, time domain <br />reflectometry (TDR) data, and borehole geophysical data. The annual <br />subsidence report shall also include a map ofsurface monument locations, <br />locations of wells where TDR and borehole geophysics were conducted, a <br />list of monument and well coordinates, and a summary ofpertinent <br />observations during the reporting period. <br />(~ Injection fluid and production fluid.• (see Section 4.0, Process Monitoring, Water <br />Monitoring Plan (Appendix I)). Pipelines shall be equipped with sampling oorts <br />to facilitate periodic collection of samples for chemical analysis. Injection fluid <br />samples shall be collected from the pipeline immediately upstream of where the <br />pipeline branches to feed all solution mining wells. Production 11uid shall be <br />sampled on the production trunk pipeline prior to entering the steam stripper. <br />Samples of both the production and injection fluid will be collected and analyzed <br />according to Section 4.0 Water Monitoring Plan Appendix I, for WQ2 <br />Parameters, Table 3. <br />(h) Monitoring of water quality and water level (see Section 2.0 of the Water <br />Monitoring Plan Appendix I) in all well completions above the Dissolution <br />Surface will occur on a monthly basis during the baseline period. The <br />completions above the Dissolution Surface will be sampled for water levels and <br />analyzed for the constituents on the WQ2 Table 3 list, page 12 of Appendix I. The <br />Dissolution Surface completions will be sampled monthly for water level, <br />conductivity and temperatures. <br />*** <br />F. FINANCIAL RESPONSIBILITY <br />Demonstration of Financial Responsibilitp. The permittee is required to <br />maintain continuous financial responsibility and resources to close, plug and <br />abandon the injection well as provided in the plugging and abandonment plan. <br />(a) American Soda has chosen to demonstrate financial responsibility through a <br />Standby Trust Agreement and a SAFECO Insurance Company ofAmerica Surety <br />Performance Bond (issued to American Soda, L.L.P., the well operator), in the <br />amount of $68, 400. This bond will increase as required by the addition of new <br />wells. The Performance Bond names EPA as beneficiary in the event ofpermittee <br />default on the plugging and abandonment requirements and is hereby <br />incorporated as part of this Permit. The Standby Trust Agreement established by <br />the permittee shall remain in effect for the duration of this Permit, unless part (b), <br />below, has been complied with. <br />(b) The permittee may, upon written request to EPA, change the type of financial <br />Draft Modification No. 9 UIC Area Permit No. C030858-00000 Page 5 of 44 <br />
