results required by Section G (3)(a) in the quarter following the date of <br />demonstration. <br />The first Quarterly Report shall cover the period from the effective date of the <br />Suspension Notification through the end of that quarter. Subsequent Quarterly <br />Reports shall cover the periods of: January 1 through March 31; April 1 through <br />June 30; July 1 through September 30; and, October 1 through December 31. Each <br />Quarterly Report shall be submitted to the EPA Denver Office thirty (30) days after <br />the end of the quarter to the address below. Temporary Plugging: The borehole and casing of wells placed into temporary suspension <br />status must be securely plugged to prevent any movement of the fluid into or between <br />USDWs. <br />a. Temporary Plugging Plan. All wells to be placed in temporary suspension status <br />are deemed to be candidates for future operation. Any wells that are not deemed to <br />be candidates for future operations shall be plugged and abandoned using agency <br />approved methods in accordance with Part II Section E of this permit. The <br />permittee must temporarily plug suspended wells according to the following plan. <br />1. Immediately prior to temporary plugging, the tubing system shall be <br />removed from the intermediate casing. <br />2. A cast-iron bridge plug (CIBP) shall be set inside each string of casing at <br />approximately the elevation of the bottom of the short string casing. If it <br />is not possible to run a CIBP inside each string of casing due to <br />irregularities, acement basket shall be run inside the casing to establish a <br />cement plug. The bridge plug or cement basket will be set at least 25 feet <br />below the Dissolution Surface. If the bridge plug or cement basket <br />cannot be set at least as deep as 25 feet below the Dissolution Surface, the <br />permittee shall Tile an approvable well repair plan with the Director <br />within 30 days or the well shall be plugged and abandoned per an EPA- <br />approvedplugging and abandonment plan within 60 days. <br />3. The well bore will be filled with a conditioned 101b. bentonite gel or <br />equivalent. An oil blanket may be used at the surface to prevent freezing. <br />b. After completion of temporary well plugging, an evaluation will be made as to <br />whether any partial or complete reclamation of affected well sites and roads should <br />be conducted. Reclamation and re-vegetation will be conducted as prescribed by <br />BLM and CDMG Permit requirements and as influenced by weather factors. <br />c. Temporary Plugging Report. Within sixty (60) days after plugging the well, the <br />permittee shall submit a plugging completion report to the Director.
