additional waste disposal site was never utilized. The Division conditionally added <br />$8,200.00 to the bond amount, which was to be posted when the disposal site was developed. <br />Since the site never was developed, the bond amount remained at $55,114.00. <br />Technical Revision No. 4, approved March 23, 1993, modified the emergency spillways on <br />both the upper bench sedimentation pond and the lower pond. <br />Technical Revision No. 5, approved December 28, 1993, responded to midterm issues. <br />Among the issues resolved were well permit application for the mine dewatering pump, <br />thereby satisfying Stipulation No. 3, renewal and update of the NPDES permit, and a <br />stipulation requiring the submittal of a reclamation plan. <br />Technical Revision No. 6, approved December 14, 1993, revised the plan for the refuse pile <br />in response to Notice of Violation No. C-93-104. The refuse pile has been regraded to a <br />3H:1V slope, covered with two feet of approved materials, and has been redesigned to <br />incorporate a peripheral drainage ditch around the pile, which has geomat and riprap at the <br />outlet. The pile now has positive drainage to the upper bench sediment pond. Additional <br />berm construction at the toe has also been implemented. Seeding and mulching will occur <br />within the next appropriate season. <br />Technical Revision No. 7, which is the response to Stipulation No. 16, has been approved by <br />the Division. TR-07 is primarily a detailed analysis of the final reclamation plan, including <br />the surface drainage, sediment pond sizing, removal of the Lewis Gulch Fill, topsoil <br />distribution, final topographic contours of both the mine and sales sites, status of the <br />structures, post-mining land use, and permit updates. <br />Minor Revision No. 5, approved September 10, 1991, was for verification of closure of the <br />exploration drill holes. The operator submitted abandonment reports for the drill holes. <br />Minor Revision No. 6, approved July 16, 1993, modified ditch construction specifications for <br />the mine access road at the portal area. <br />Minor Revision No. 7, approved September 10, 1992, updated the permit's legal compliance <br />status by identification of owners, page 2.03.4(1). <br />Reclamation Plan <br />Since the mine closed and ceased operations in August 1993, the mine has completed several <br />reclamation tasks. These tasks include: backfilling of the portals with 25 feet of fill <br />material; removal of much of the mine equipment, and partial elimination of the bone yards <br />at both the portal and sales site; removal of the warehouse infrastructure; grading of the road <br />from the refuse pile to the sediment pond for drainage and berming of the same road; <br />decommissioning the mine discharge pump; and removal of 2,000 cubic yards of overburden <br />from the Lewis Gulch Fill to reclaim (backfil] and grade) the refuse pile. <br />With the submittal of Technical Revision No. 7, the operator has committed to reclaiming <br />the site in the following manner: <br />13 <br />
