Pages V-48 and 49 - These two pages are missing from the Division
copies of the permit application.
Table V-4 - This table could not be located within either of
the Division copies of the permit application.
Table V-5 - This table should be revised to add the rail loop
and slot storage areas as well.
7. Miscellaneous Revisions to Maps and Illustrations.
In General, Western Fuels should review all maps within the permit to ensure
that the as-built configuration of a given structure or facility is accurately
depicted on the map. Some specific changes to be made to particular maps are
described below:
Map 85 - Ventilation Area Site Preparation. This map should be revised to
show the as built configuration of the facility, including runoff
collection ditches, culverts and other sediment control measures.
Map 103A - Site plan railroad loadout loop. This map should be revised to
show the two added sediment ponds, fill slope runoff collection
ditches, and the present status of the staging areas within and to
the south and east of the rail loop. If the staging areas have
been reclaimed, the outline of the reclaimed areas should be
delineated on the map.
Map 105 -Site Plan and Reclamation Plan Coal Storage Area. This map needs
to be revised to show the as-built configuration of the facility,
including the location and configuration of the sediment ponds,
runoff collection ditches and topsoil stockpiles.
Map 108 -Overland Conveyor and Maintenance Road Plan and Profile - Sheet 3.
This map should be revised to show the current status and
configuration of the two staging and laydown areas near the County
Road.

Revise in the same fashion as map 108.

It is unclear from the information provided on the map as to the
number and type of animal crossings installed along this section of
the conveyor route. Please clarify.

Revise on the same fashion as Map 108.
Illustration 13 - The copy of this illustration in the permit application is
illegible and should be replaced with a new copy.
