Pikeview Quarry -Decision Notice and FONSI <br />08/30/01 <br />Under the regulations of the Council on Environmental Quality (CEQ) for <br />implementation of the NEPA, an Enviromnental Assessment (EA) was prepared to <br />determine whether the proposed action may cause significant environmental impacts (40 <br />CFR 1500). The EA identified and evaluated three alternatives including the no action <br />alternative as well as the proposed action and an enhanced reclamation alternative. This <br />Decision Notice describes the Forest Service evaluation of these alternatives and <br />responds to Castle Concrete's proposal. <br />DECISION <br />This decision regarding the layback of the Pikeview quarry is based upon review of the <br />EA, supporting materials referenced in the EA, and additional information contained in <br />this Decision Notice <br />I hereby authorize Castle Concrete to layback the Pikeview quarry in accordance with the <br />mining and reclamation plans and timeframe delineated in Alternative B. A Special Use <br />Permit for the Pikeview quarry will be issued as a result of this decision and will include <br />the mitigation and monitoring measures detailed under Chapter 2 and Chapter 5 of the <br />EA. Implementation of Alternative B including the proposed mitigation and monitoring <br />measures ~ be in accordance with the standards and guidelines contained in Chapter III <br />of the Forest Plan. <br />The following permit conditions must be considered and met by the applicant: <br />1 Any deviation from the mining and reclamation plan described in the EA is <br />subject to review and approval by the Forest Service and the Division of Minerals <br />and Geology (DMG). If necessary, the DMG will change the amount of the bond <br />in accordance to changes to the reclamation plan. <br />2. The layback of the Pikeview Quarry must comply with the performance standards <br />and molitoring plan described in the EA. <br />3. The applicant is responsible for addressing the potential impacts of the proposed <br />layback on the drainage east of the quarry discussed in the EA. Castle Concrete is <br />responsible for assuring that measures aze taken to prevent further impacts on this <br />drainage. <br />This decision is subject to approval by the Division of Minerals and Geology, <br />State of Colorado, Department of Natural Resources, by the establishment of a <br />reclamation bond sufficient to ensure that reclamation will take place, including <br />the affected areas on Forest System lands. The most recent bond was dated <br />November 1, 1994 in the amount of $604,244.00. This amount may need to be <br />adjusted according to the layback plans. <br />
