This authorization is for a Class III Solution Mining Well(s) Area Permit to extractthe
mineral Nahcolite (NaHC03) by use of a solution mining process with elevated temperature and
pressure. Prior to receiving approval to construct additional wells as authorized by this Permit,
the applicant must submit the information required by 40 CFR § 144.33.
Solution mining activities (operations) shall not commence until the Permittee has
fulfilled all applicable conditions of this Permit and has received written authorization from the
Director. "Prior to Commencing Injection" requirements are set forth in Part IT, Section C. 1. of
this Permit.
Atl conditions set forth herein refer to Title 40 Parts 124, 144, 146, and 147 of the Code
Minor Modification No. 8 UIC Permit No. C030858-00000 Page 3 of 8
