Solution mining activities (operations) shall not commence until the Permittee has <br />fulfilled all applicable conditions of this Permit and has received written authorization from the <br />Director. "Prior to Commencing Injection" requirements are set forth in Part II, Section C. 1. of <br />this Permit. <br />All conditions set forth herein refer to Title 40 Parts 124, 144, 146, and 147 of the Code <br />of Federal Regulations and are regulations that are in effect on the date that this Permit becomes <br />effective. <br />*** <br />PART II. SPECIFIC PERMIT CONDITIONS <br />**~ <br />D. MONITORING, RECORD KEEPING, AND REPORTING OF <br />RESULTS <br />MONITORING: <br />The Permittee is required to cam out the activities described in the monitoring plan. The <br />data must be reported and maintained as provided in Section II D.4 of the Permit. The 4`h <br />Revision of the Water Monitoring Plan, dated December 23, 1999, has been incorporated <br />into the Permit as Appendix I. <br />The Water Monitoring Plan includes ground-water monitoring, for the Piceance Site <br />(solution well field and upper plant) and the Parachute Site (lower plant, which is not <br />covered by the Permit). It also contains a process monitoring section, (found in Section <br />4.0 of the Water Monitoring Plan, Appendix I) for mining operations at the Piceance Site. <br />We1120-12 (EPA Permit No. CO 30858-04552) shall be converted to monitoring <br />status in a manner that shall isolate the mining cavity and allow the well above the <br />cavity to be converted to a time domain reflectometry (TDR) and temperature <br />monitoring well. The conversion plan for we1120-12 shall be as follows: <br />+ A cast iron bridge plug (CIBP) shall be set inside each string of casing at <br />approximately the elevation of the bottom of the short string casing. If it is <br />not possible to run a CIBP inside each string of casing due to irregularities, a <br />cement basket shall be run inside the casing to establish a cement plug at <br />approximately the elevation of the bottom of the short string casing. <br />Minor Modification No. 8 UIC Permit No. C030858-00000 Page 7 of 8 <br />
