Please consider and <br />amend the narrative if appropriate to limit the commitment to sedimentation ponds and other sediment <br />retention structures. <br />21. Text on page 5 of Tab 13 states that "all sedimentation ponds will be inspected quarterly under the supervision <br />of a registered professional engineer and reported to the DMG." Please amend the text to clarify that all <br />impoundments (not just sedimentation ponds) will be inspected and reports will be submitted quarterly. <br />Also, please amend the text to commit to annual P.E. certification of all impoundments. <br />22. Text on page 5 of Tab 13 states that "permanent impoundments will be reevaluated prior to bond release and <br />remedial construction performed to satisfy the appropriate sections of the regulations". Specific <br />demonstrations required for permanent impoundments are addressed in Rule 4.05.9(13). Please amend the <br />referenced text to specify that, prior to submittal of a bond release request for a permanent <br />impoundment, compliance with the requirements or Rule 4.05.9(13) (a) through (f) will be <br />demonstrated. <br />23. Attachment 13-8 addresses design plans and sizing calculations for small azea exemption catchment sumps <br />associated with specified topsoil stockpiles. In addition, the Attachment includes a commitment to certify <br />catchment sumps following constmction. The Attachment also contains documentation for stockpiles which <br />do not require catchment sumps due to satisfactory vegetation. The attachment was last updated in November <br />1993. Since that time, a number of additional stockpiles have been approved and constructed, some of which <br />do not drain to sediment ponds. <br />Please amend Attachment 13-8 as appropriate to identify all existing and approved topsoil stockpiles <br />that do not drain into sedimentation ponds, and provide appropriate small area exemption designs and <br />demonstrations. For all existing stockpiles that utilize catchment sumps for sediment control, please <br />provide copies of the construction certifications and current annual certifications. <br />24. A Pond 16 design revision was submitted in PR-3, Exhibit 13-9B. The approved version was certified 4/22/03. <br />There are several design changes on the "Design Revision", as compared to the "As-Built" dated 9/26/01. <br />Among these changes are modified Emergency Spillway Schedule, Principal Spillway 2.5 feet lower than "As <br />Built" on the Stage Capacity Chart and Principal Spillway Detail drawing, and de-watering orifice specified on <br />"As-Built" is not shown on design revision. <br />Please confirm whether these design modifications have been completed. [f so, please provide a revised <br />construction certification and "AS-Built". Please amend the NPDES Monitoring narrative on page 10 of Tab 15 to update the status of Pond 17 <br />and associated NPDES discharge site. <br />26. There is narrative reference to the five W-24 series Sage Creek/Wolf Creek monitoring wells on page 1 l of <br />Tab IS and Appendix IS-3A (page IS-3a-2). Please update these sections to described the damage <br />sustained by these wells in 2003, the current status of the wells, and the current plans for abandonment <br />and replacement. <br />27. The Cnrface Warer (7na~ narrative section on page 3 of Tab 16 is outdated. Please amend the section to <br />reBect the construction of Sediment Ponds OtS, 016, Ot6A, and 017. <br />19 <br />
