PART II -SUMMARY OF: OPERATIONS, REVISIONS TO PERMIT, <br />VIOLATIONS, STIPULATIONS, VARIANCES, AND BOND <br />Revisions to Permit <br />Table 2, below, summazizes permitting actions for Permit C-92-080 that have occurred since <br />the last permit renewal RN-2, approved May 29, 2003. <br />Table 2 -Permitting actions since last renewal of permit C-92-080 <br />Approval Date Permit Action <br />10/9102 Technical Revision 11, modification of the reclamation plan. <br />7/8/04 Technical Revision 12, modification of the reclamation <br />plan and the permit boundary. <br />5/23/05 5L-2, Phases I, I, and III release of South Pit area. <br />Violations <br />No Notices of Violation have been issued since the last permit renewal. <br />Permit Stipulations <br />Permit C-92-080 has no active stipulations. The most recently active stipulation, ST-9, was <br />complied with and removed from the permit on July 27, 2004 after the permiee submitted the <br />Technical Revision and bond release application required by that stipulation. <br />Bond <br />DMG is evaluating the reclamation liability at the Cazbon Junction Mine in Technical <br />Revision application TR-13, currently in review. <br />PART III -COMMENTS AND REQUESTED REVISIONS <br />OF PERMIT APPLICATION <br />Based on the current Midterm Review, the Division Snds that permit application C-92-080 <br />needs updating, as described below. As a result of this finding the Division requests that the <br />permiee submit revised permit pages in a Technical Revision by March 1, 2006. <br />1. We cannot find in the permit application a map showing the proposed postmining <br />topographic contours for the portion of the permit azea that is south of the latitude of <br />pond 2.
