The following responses indicate what is proposed for document updates and revised pages are attached for your approval. To assist in your review, added information in the text has been bolded. Upon approval, pages with the bold removed will be submitted. I. Page 3 of the DMG Application Form has been updated to reflect current conditions at the site. 2. Kaiser submitted an Operator Name Change notice (Revision dated 6/30/95) to the Division which was placed in the front o(the permit document (copy attached). This notice advises the reader of the name change from Kaiser Resources, lnc. to Kaiser Ventures, Inc. and explains that the change has nol been made throughout the document. Information concerning corporate officers and their positions have been updated on revised pages 2.03-3 and 2.03-4. 3. Kaiser has not acquired any other coal mining or reclamation operations in the United States and, therefore, no updates to the permit document are necessary. 4. Page 2.03-9 of the permit document has been updated to include any NO VS issued since April 15, 1993 at coal mines owned or controlled by Kaiser. 5. Page 2.03-10 of the permit document has been updated to reflect that the original permit was issued on January 7, 1983 and that the current permit was approved on April 20, 1993. 6. Page 2.03-12 of the permit document has been updated to re0ect the current DMG permit and Department of Health NPDES permit. 7. Both surface and groundwater are currently being monitored at the site and the results are updated each year in the Annual Hydrology Report submitted to the Division. Rather than include information in the permit document which would need to he updated each year, Kaiser has referenced the AHR on revised Page 2.04-13 for groundwater and is already referenced for surface water on page 2.1k3-18a.
