The following responses <br />indicate what is proposed for document updates and revised pages are attached for your approval. To assist <br />in your review, added information in the text has been bolded. Upon approval, pages with the bold removed <br />will be submitted. <br />I. Page 3 of the DMG Application Form has been updated to reflect current conditions at the site. <br />2. Kaiser submitted an Operator Name Change notice (Revision dated 6/30/95) to the Division which was <br />placed in the front o(the permit document (copy attached). This notice advises the reader of the name <br />change from Kaiser Resources, lnc. to Kaiser Ventures, Inc. and explains that the change has nol been <br />made throughout the document. Information concerning corporate officers and their positions have been <br />updated on revised pages 2.03-3 and 2.03-4. <br />3. Kaiser has not acquired any other coal mining or reclamation operations in the United States and, <br />therefore, no updates to the permit document are necessary. <br />4. Page 2.03-9 of the permit document has been updated to include any NO VS issued since April 15, 1993 <br />at coal mines owned or controlled by Kaiser. <br />5. Page 2.03-10 of the permit document has been updated to reflect that the original permit was issued on <br />January 7, 1983 and that the current permit was approved on April 20, 1993. <br />6. Page 2.03-12 of the permit document has been updated to re0ect the current DMG permit and <br />Department of Health NPDES permit. <br />7. Both surface and groundwater are currently being monitored at the site and the results are updated each <br />year in the Annual Hydrology Report submitted to the Division.
