Cattany <br />Atting Division Director <br />The Division has reviewed Trapper Mining, Int.'s combined application for a Permit Renewal No. 4 <br />(RN-OS) and Permit Revision No. 4 (PR-04). After initial review, the Division has the following <br />concerns. <br />Rule 2.05.3(6)(a)(v) requires DMG mining and reclamation permits contain a description <br />of the pern3ittee's procedures for recording and reporting to the Division the results of any <br />required pre-blast surveys. Please update the TMI permit, perhaps in the form of a new Exhibit, <br />to provide the following information: <br />(a) the names and addresses of all residents and owners of dwellings situated within, <br />and within one-half mile beyond, the TMI permit boundary; <br />(b) the names and addresses of all owners of structures other than dwellings situated <br />within, and within one-half mile beyond, the TMI permit boundary. Structures such as <br />these would include, but not be limited to, schools; churches; hospitals; nursing facilities; <br />community or institutional facilities; commercial or industrial facilities; barns; silos; <br />garages; storage sheds; water wells; water or sewage lines; concrete irrigation headgates; <br />concrete culverts and bridges; propane or butane tanks; oil or gas wells; oil or gas <br />collection, storage, or transmission facilities and pipelines; and concrete footers or <br />foundations for electrical transmission towers and telecommunication antennas. <br />Structures other than dwellings situated within the TMI permit boundary used in support <br />of operations need not be included; <br />(c) the locations of these structures on a map, or statements as to which existing map(s) in <br />the TMI permit these structures aze currently depicted upon; <br />(d) the dates pre-blast survey notifications were sent to the residents and owners of these <br />structures; <br />C-1981-010 RN04/PROS 1 08/29/02 <br />
