ATTACHMENT A <br />Logan Wash Mine <br />Permit No. M-1977-424 <br />Amendment No. 1 CDRMS Adequacy Issue Response <br />I: Please either submit BLM approval of the plan to weld the covers on the manholes <br />or revise the reclamation plan to provide for removal of the manhole collars to a <br />minimum 2 feet below ground surface, backfill the hole with inert fill, replace topsoil <br />and revegetate per the plan specifications. <br />Response: <br />An approval letter was requested and received from the BLM. Although the BLM <br />initially verbally approved of Amendment No. 1, as a result of the DRMS concern on the <br />manhole collars, the BLM has asked that OOSI backfill all manhole vaults and remove <br />any above-grade cement collars, OOSI notes that the pipeline corridor is 100% reclaimed <br />with growth of shrubs in some areas exceeding 6 feet in height. Reclamation of the <br />already buried manholes (as a result of natural sedimentation and deposition of <br />colluvium) as requested by DRMS and BLM will potentially destroy this vegetation and <br />delay closure of the mine permit for a number of years after the manholes are backfilled. <br />The revised reclamation text is provided in Attachment B. <br />2. Please include a commitment to submit results of TCLP tests on the manlrole sump <br />sediments, evaporation pond precipitate, and pond/pipeline water (if any to be disposed <br />o,~ and secure DRMS approval of methods for disposal, prior to initiating clean-up and <br />disposal operations. <br />Response: <br />OOSI will make a revision to text in Exhibit E (Reclamation Plan), that commits to <br />conducting a TCLP test of manhole sump sediments, evaporation pond precipitate, and <br />pond/pipeline water prior to the disposal of these media; and securing DRMS's approval <br />of disposal methods prior to initiating clean-up and disposal operations. The revised <br />reclamation text is provided in Attachment B. <br />3. Please provide proof of publication and mailing notices to all landowners on or <br />within 200 feet of the proposed affected area. <br />Response: <br />All relevant land owners have been contacted by phone and facsimile. A copy of the <br />letter has been faxed to them for their signature. Acknowledgment of receipt of the <br />mailed notice through a signature and date have been provided by all landowners and is <br />presented in Attachment B. <br />
