Destructive extrusion seam samples DX-17 through DX-25 were marked, removed, and tested. <br />Polyflex completed geomembrane construction for the year and demobilized from site on November <br />30~'. <br />Ames began placement of Low volume solution collection piping on November 12u'. Ames placed fl- <br />inch PCPEpipe in the locations defined by the drawings. <br />Ames commenced placement of Low Volume Solution Collection Fill (LVSCF) in Phase IVb on <br />November l lu' by building 10-foot haul roads into the Phase IVb PSSA. <br />Ames re-excavated the PSSA sump and placed Soil Liner Fill (SLF) as requited prior to the <br />installation of geosynthetics. Ames excavated the LVSC riser trench and performed anchor trench <br />excavation activities after all SLF was in-place. Ames conducted five Depth Verification Tests <br />(DVT) SLF that was placed in the Phase IVb sump area. <br />Ames continued backfill of the interior anchor trench along the 9606 road on the Phase III ore slope at <br />the completion of geomembrane QA testing. Ames bacldilled approximately 700 linear feet of anchor <br />trench. <br />Phase IVc <br />Ames performed excavation remediation activities for underground workings UNK-10, UG-234a, <br />UG-243, UG-236, UG-506, and UG-546. <br />Ames performed co~rmatory drilling activities associated with the Phase Nc construction area on <br />underground workings UG-304, UG-498, and UNK-18. <br />Ames placed Structural Fill (SF) over previously remediated workings at UG-227, UG-511 and UG- <br />512 and installed Coazse Shaft Backf3ll (CSB) at UG-22], UG-498, and UG-304: <br />Ames placed Concrete Plug materials (CP) at UG-221, UG-500, UG-502, UG-503, and UG-506 <br />Ames performed blasts at UG-297 and UG-234. <br />Field Monitorine and Testine Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. Golder observed Structural Fill (SF) placement in Phase N for materials, lift <br />thickness, moisture content, and compactive effort. <br />Golder observed and documented excavation remediation activities associated with the Phase IV <br />underground working construction.
