<br />minor disturbance as defined by Rule 1.1(13). <br />On a map labeled Exhibit E - Mining Plan Map, outline all <br />major surface features including the pit boundary, topsoil <br />stockpiles, overburden stockpiles, product stockpiles, and <br />processing areas (the current permit allows no on-site <br />processing so please make that change if you intend to do so <br />in the future) . Show where the mining will start and the <br />direction it will proceed. Also label the surface owners <br />within 200 feet of the affected area on the map. <br />5. On a separate map labeled Exhibit E - Reclamation Map, show <br />the gradient of all reclaimed slopes sufficient to describe <br />the post mine topography. State the average thickness of <br />overburden and topsoil to be replaced by reclamation area or <br />phase. <br />6. If topsoil is to be stockpiled for a period greater than one <br />year, you will need to describe how it will be stabilized with <br />a vegetative cover. <br />7. Include a statement that the applicant intends to file or has <br />filed for a stormwater permit with the Colorado Department of <br />Health. If you have done so already, would you please send us <br />a copy? <br />8. Before mining in any areas with potential wetlands or into <br />existing unnamed tributaries, the applicant will first need to <br />acquire clearance with the Army Corps of Engineers. <br />9. If a clearance has not been obtained from the State Historical <br />Preservation Officer at the time of approval, the Division <br />will condition the amhndment approval such that no disturbance <br />of uncleared areas may occur until a copy of the SHPO release <br />is received. <br />Rule 1.8.1 states that changes and additions to the permit <br />application must be filed with the Division and with the County <br />Clerk and Recorder. The applicant shall also provide the Division <br />with an affidavit or receipt demonstrating that the change was <br />filed with the County Clerk and Recorder not later than the close <br />of business day the change was filed with the Division.
