Please provide response prior to <br /> August 24, 1992 . You may request a continuance of this application if you can <br /> not respond by the above date. <br /> EXHIBIT C-1 Pre-Mining Plan Map <br /> 1 . The map shows two wells or other labeled as pumps on the North East <br /> portion of Phase 1 and Phase 3. Are these the only wells man made <br /> structures located with in 200 feet of the permit area? <br /> 2. Rule 2 . 1 .2( 1 )(b) - all maps in the application must be signed by a <br /> registered land survey, professional engineer , or other qualified person . <br /> 3. Mining Plan map C-1 shows a gas line and a buried telephone cable , please <br /> submit the agreement you have with the operators of gas and telephone <br /> line in the event the operation impacts the lines . <br /> EXHIBIT G. Water Information <br /> 1 . On page G-1 paragraph II you stated that three culverts will be installed <br /> between the ponds to allow proper drainage and groundwater flow of the <br /> site. If any discharge takes place an NPDES Permit from the Health <br /> Department is required. There was no indication in amendment application <br /> of the need for an NPDES permit, please clarify. You stated that <br /> discharge from dewatering the site will be monitored in accordance with <br /> the discharge permit to be obtained. An NPDES Permit is a Pre-Requisted <br /> for the operation to be approved. <br /> 2 . On the second paragraph you stated that the total water requirements for <br /> the site will be approximately 1 .2 acre feet. This revision has received <br /> a letter from the State Engineers office which states that the "operator <br /> had applied for a Water Augmentation plan 1989 but the well permit <br /> appl 'cations were returned in 1990 as unacceptable and ha•:a never been <br /> refiled with this office. " Before the start or the operation a Water <br /> Agumentation plan has to be submitted. On page G-1 paragraph S you <br /> stated that all permitted wells in the area are sncwn of rigure G-1 and <br /> listed on Table G-1 . There was no figure G-' submi _ ,?d ai ;c t:.-e <br /> amendment please submit figure 3-1 . <br />
