An application for all types of revisions or a permit shall contain: *Rule 2.08.4(5)(a) An identification of the permit by permit number or other appropriate reference which is the subject of the revision; The application is in compliance with this mle. An application form accompanied the technical revision request that identified the permit by permit number C-81-019. *Rule 2.08.4(~(b) A specific description of the requested change in the terms of the permit; The application is in compliance with this mle. Specific descriptions of requested changes were provided by submission of revised pages. *Rule 2.08.4(5)(c) A specific description of any changes in the mining and reclamation operation which may have an effect on performance bond requirements of Rule 3; and The application is in compliance with this rule. The application cover indicates no change in the bond requirement. *Rule 2.08.4(5)(d) Such other information as may be necessary for the Division to determine if the proposed revision will comply with the Act and these Rules. The Board or Division may reasonably request additional information to evaluate the proposed revision. The application is in compliance with this rule. Three copies of the application were provided as required by the Division. Rule 2.08.4(6)(b)(i): An applicant for a technical revision shall place an advertisement in a local newspaper pursuant to the requirements of 2.07.3(2), except that the publication need only be once and no reference to an informal conference shall be made. j The applicant shall make a copy of his or her complete application for the public to inspect and copy. z This shall be done by filing a copy of the application with the recorder at the courthouse of the county closest to the operations, or if approved by the Division, at another equivalent public office, if it is determined that the office will be more accessible to local residents than the county courthouse.2 The public copy shall be filed at the same time as the advertisement.? ~ The application is in compliance with this rule.
