MEMO to 11Tnerals Stajj' <br />February 8, 2002-Toxic and Hazardous Spill Reporting (printedApril3D, 2002) <br />Page 2 - - <br />2, i3'hen a reportable spill is reported to the Office, staff shall obtain the following information <br />from the;operator: <br />• -Operation name, peri3rit nurriher and name of person reporting the spill. <br />• Date andaime of spill. <br />• Type of material spilled (CAS number if applicable, from the IvfSDS form), amount spilled, <br />lateral extent of spill, whether any material has leR the permit area, and where the spilled.- - - --_- <br />material migrated to. <br />• What measures are being and/or have been taken to contain the spill and clean it up. <br />• Have the appropriate.local,. state, federal authorities been notified. Request the operator <br />submit a copy of any required written reports sent to other agencies. <br />--•---mat-if~ny,-mitigation-measures were required-by the otherjui-isdictiohs-under their-- - <br />authorities. Request the operator send the Office a copy of any required written report sent to <br />other agencies. <br />3. Any reportable spill of toxic or hazardous material will be handled on a ease-by-case basis; as faz <br />as the need for Boazd action. <br />4. The Office shall conduct a mine site inspection dependent upon site conditions and the probable <br />risk of contamination to ground water from the spilled ma#erials.
