Engineering designs including longitudinal profiles, cross sections, and plan views of all <br />channels, ponds, culverts, spillways, or other hydraulic structures, as necessary. <br />4. Engineering design(s) and construction techniques of any alternative sediment and/or erosion <br />control structures other than those specified in item 3 above. <br />5. A list of all property owners that may be affected by the proposed stormwater management <br />plan and their necessary approvals. <br />6. A commitment to design and build all proposed stormwater controls such is channels, <br />ponds, and spillways in such a way as to minimize impacts to the hydrologic balance within <br />the quarry as well as the surrounding area. <br />7. A commitment to have all proposed structures installed and functioning prop~;rly no later <br />than October 31, 1996. <br />8. Contingency plans for a discharge of excessive stormwater flows offsite onto 'Iirnnel Drive <br />(i.e., a 100-year storm event or greater). <br />9. A commitment to have the construction of all proposed structures certified by a registered <br />professional engineer, <br />10. A schematic which shows the flow path of all nunoff water from the upper reaches of the <br />disturbed area, along the haul road, through the various containment ponds, ro the lower <br />reaches of the disturbed area. <br />11. A discussion of which structures will be left in place during final reclamation, which <br />stnrctures will be removed, estimated construction costs of any temporary stnuctures, and <br />any plans for stream channel reconstruction during final recLunation. <br />12. A commitment to cease and desist from any disturbance in the amendment ana, until the <br />operator has received Division approval of an acceptable plan for minimizing impacts to the <br />hydrologic balance in the quarry and surrounding area during management of stormwater <br />runoff and has submitted certifications from a registered professional engineer that all <br />designed structures have been constructed and are functioning properly. <br />M:\oe~\TVak.964 <br />
