Check for $ 1938.00 (amendment fee); <br />2. Original and copy of the completed application form; <br />3. 2 copies of Exhibits A - S <br />Due to the issues with the "lost Original Bond" documents, the permit that this application <br />will amend is currently inactive until the December 14, 2006 IyfLRB Hearing. I will be at the Hearing to explain <br />to the Board about the lost bond and this pending amendment application. The Board should then be able to <br />make a derision and issue (activate) blr. Gxay's permit on that date as the 14'" is 20 days after my pubhc notice <br />as to the re-issue of the Bond. Russ Means said that I could send this amendment application to you so you <br />could go over it and see that it is complete and has no deficiencies so that if/when the Board decides on 14 <br />December, you could receive bir. Gra}''s application on that date and stazt the 90 day review period. <br />Let me know if there axe any deficiencies. ds this is the same document which has been submitted and <br />accepted "twice" previously it should be complete.
