On page 2.03-12(j) the Kanawaha operation is missing an FEIN. <br />4. The addresses for the following parties need to be included under the <br />approprate section for land ownership: <br />a. Middle Creek Subdivision <br />b. William Rogers <br />c. Dennis Hayes <br />d. B.J. Cornish <br />5. Page 2.03-23 indicates TCC has only two violations during the last three <br />years but four are listed. <br />6. Is a special use permit for the eastern mine expansion required by Routt <br />County? If so, then text committing to submission of this information to <br />the DMG should be added. <br />Rule 2.04.3 Site Description and Land Use <br />1. For the record, the Division understands and concurs with pastureland as <br />the pre-mining land use for the two surface disturbances proposed in the <br />Eastern Mining District. However, much of the steep rocky dip slopes of <br />V~k the Twentymile Sandstone are not currently pastureland but are rangeland <br />Was f+~Y'~2 la,~by definition. This should not affect the post-mining land use of <br />pastureland for the two small surface disturbances proposed. <br />2. As required by part (3) of this section, please comply with (a)-(f) for <br />previously mined areas within tfie proposed permit area and update Table 6. <br />Rule 2.04.4 Cultural and Historic Resource Information <br />1. Attached please find correspondence received from the State Historic <br />Preservation Officer. Please submit a map as required by 2.10.3(1)(8) at <br />the same scale as the Mine Plan map, Expand the commitment on page <br />2.04-11(c) to include surveying of all areas affected by subsidence in <br />addition to surface facility disturbed areas. Also, the BLM survey <br />information needs to be submitted as part of this application. <br />Rule 2.04.5 General Description of Hydrology and Geology <br />1: Map 4, Proposed Area Hydrologic Boundaries should be updated to show the <br />proposed mine permit boundary. <br />2. The water inflow calculations for the Eastern Mining District are not <br />legible., Please send clean copies.. Also, Exhibit 9A needs some <br />explanation in the text as to what information is being supplied and how <br />it applies to Rule 2.04.5. <br />
