~.wu~ttuu Lc~a~uucu~ ui ruviw iacttiw ttuu uuvuwuucu~ <br />Air Pollution Contol Division <br />- MINING OPERATIONS - <br />GENERAL INSTRUCTIONS FOR <br />FUGITIVE PARTICULATE AIIi POLLUTANT EMLSSION NOTICE /CONTROL PLAN <br />An Air Pollution Emission Notice and Fugitive Particulate Control Plan must be submitted with an application for an <br />Emission Pemrit Completion of this form will meet these requirements. Complete only those sections of this form <br />which are applicable to your operation All data given, such as production rates, topsoil removal, etc, should be for <br />that period of 12 months that is expected to have the greatest amount of activity, usually for the first year. Emissions <br />will be calculated in terms of pounds per day and tons per year of particulates generated Also, please submit the <br />following, if available: <br />1. A map showing site location and boundaries, haul roads, and nearby residences or commercial structures. <br />2. Production schedule. <br />Schematic of the operation. <br />4. Engineering or manufachuing data on control equipmenrt. <br />Fugitive Particulate Emission Pemrits are issued for site-specific activities, such as mining, storage ofmaterials, haul <br />road activities, etc. <br />The Division will use the information submitted on this fomr to estimate emissions from the activity. <br />Any processing equipment, such as a crusher, screen, concrete batch plant, or asphalt plant is considered to be <br />portable and requires a separate pemut application, Air Pollution Emission Notice, and filing fee. <br />This aids the applicant by requiring only a revised Air Pollution Emission Notice for Relocation, whenever the <br />equipment is moved to a new site. <br />FEES <br />A filing fee of $119.96 shall accompany each Air Pollution Emission Notice /Control Plan fled with a pemrit <br />application Pemrit processing fees will be charged for the amount of time the Division spends evaluating the pemrit <br />application These fees, (cunently $59.98 perhour) plus arty required public notice fees, must be paidbefore an Initial <br />Approval Pemvt will be issued Once the project has commenced operation, the soumz has 180 days to certify <br />compliance with the pemvt conditions.
