Dear Ms. Abbott:
On March 11, 2002, the Division of Minerals and Geology .received your 112 Construction Materials
Reclamation Permit Amendment Application package for the Portland Limestone Quarry, Permit
No. M-1977-344. The Division could not process your Amendment at that lima since it was submitted in the
name of the wrong company and a Succession of Operators application needed to be submitted and approved.
On July 2, 2002, the Division approved the Succession of Operators Application which transferred the Portland
Limestone Quarry, Permit No. M-1977-344, from Holnam, Inc. C/O Dundee Cemem to Holcim (US), Inc.

as being submitted and technical review can begin:
APPLICATION FORM: Paee 1. Item 2, Operation Name: The Division request that you change
the name of the site on the Application form to Portland Limestone Quarry (this is the name of the site
as it is pernlitted).
APPLICATION FORM: Pages 5 and 6, Responsibilities as a Permittee: Must be completed.
EXffiBIT L -Reclamation Costs (Rule 6.4.12): (1) All information necessary to calculate the costs
of reclamation must be submitted and broken down into the various major phases of reclamation. You
must provide sufficient information to calculate the cost of reclamation that would be incurred by the
state.
