Cottony <br />Acting Division Director <br />Re: Portland Limestone Quarry, Permit No. M-1977-344, Receipt of 112 Construction Materials <br />Reclamation Permit Amendment Application Package, AM - 1 ,i <br />Dear Ms. Abbott: <br />On March 11, 2002, the Division of Minerals and Geology .received your 112 Construction Materials <br />Reclamation Permit Amendment Application package for the Portland Limestone Quarry, Permit <br />No. M-1977-344. The Division could not process your Amendment at that lima since it was submitted in the <br />name of the wrong company and a Succession of Operators application needed to be submitted and approved. <br />On July 2, 2002, the Division approved the Succession of Operators Application which transferred the Portland <br />Limestone Quarry, Permit No. M-1977-344, from Holnam, Inc. C/O Dundee Cemem to Holcim (US), Inc. <br />Today, ±he Di•rsien began re~rie~v of the Amendment Applica*acn and prelirri.^ary re~:ew of the inf nnatior. <br />received determined that the following items must be received before the Division can consider your application <br />as being submitted and technical review can begin: <br />APPLICATION FORM: Paee 1. Item 2, Operation Name: The Division request that you change <br />the name of the site on the Application form to Portland Limestone Quarry (this is the name of the site <br />as it is pernlitted). <br />APPLICATION FORM: Pages 5 and 6, Responsibilities as a Permittee: Must be completed. <br />EXffiBIT L -Reclamation Costs (Rule 6.4.12): (1) All information necessary to calculate the costs <br />of reclamation must be submitted and broken down into the various major phases of reclamation. You <br />must provide sufficient information to calculate the cost of reclamation that would be incurred by the <br />state. <br />EXIIIBIT N -Source of Legal Ri¢ht to Enter (Rule 6.4.14): Y"ou must provide the source of your <br />legal right to enter and initiate a mining operation on the affected land. This may be a copy of access <br />lease, deed, abstract of title, or a current tax receipt. <br />
