Cottony <br />Division Director <br />Natural Resource Trustee <br />Listed below are adequacy review comments for the Brownstone Quany 112 Conversion Application (CN-Ol), <br />Permit No. M-1997-065. The Division of Reclamation, Mining and Safety ("Division") is required to make an <br />approval or denial decision no later than August 13, 2007, therefore, your response to the following adequacy <br />review concerns should be submitted to the Division as soon as possible. <br />1. As required by Rule 1.6.5 (2), please submit the proof of the public notice circulated in the newspaper and to <br />all owners of record of surface lands, and easement holders that aze on the affected land and within 200 feet of <br />the boundary of the affected land. Proof of notice may be return receipts of a Certified Mailing or by proof of <br />personal service. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional engineer, or other <br />qualified person. The revised maps submitted to the Division were not signed. Please resubmit a signed copy <br />of the maps. <br />3. The Division received comments from the Colorado Historical Society and Office of the State Engineer. The <br />letters are attached. Please address the comments noted in the letters, and make any changes in the <br />application as necessary. <br />Ru]e 6.4.2 Exhibit B- Index Man <br />4. The index map needs to depict the amended area. Please revise the index map depicting the proposed permit <br />area. <br />Rule 6.4.3 Exhibit C- Pre-mining_and Minine Plan Mao(sl of Affected Lands <br />5. Please depict all adjoining surface owners of record, creeks, roads, buildings, oil & gas wells, etc.. on the area <br />of affected land and within 200 feet of all boundaries of such areas. <br />6.
