Listed below are adequacy review comments for the Brownstone Quany 112 Conversion Application (CN-Ol),
Permit No. M-1997-065. The Division of Reclamation, Mining and Safety ("Division") is required to make an
approval or denial decision no later than August 13, 2007, therefore, your response to the following adequacy
review concerns should be submitted to the Division as soon as possible.
1. As required by Rule 1.6.5 (2), please submit the proof of the public notice circulated in the newspaper and to
all owners of record of surface lands, and easement holders that aze on the affected land and within 200 feet of
the boundary of the affected land. Proof of notice may be return receipts of a Certified Mailing or by proof of
personal service.
2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional engineer, or other
qualified person. The revised maps submitted to the Division were not signed. Please resubmit a signed copy
of the maps.
3. The Division received comments from the Colorado Historical Society and Office of the State Engineer. The
letters are attached. Please address the comments noted in the letters, and make any changes in the
application as necessary.

4. The index map needs to depict the amended area. Please revise the index map depicting the proposed permit
area.
Rule 6.4.3 Exhibit C- Pre-mining_and Minine Plan Mao(sl of Affected Lands
5. Please depict all adjoining surface owners of record, creeks, roads, buildings, oil & gas wells, etc.. on the area
of affected land and within 200 feet of all boundaries of such areas.
6. Please provide the existing topography of the area with contour lines of sufficient detail to portray the
direction and rate of slope of the affected land.
