<br /> <br />PAGE TWO <br />MARCH 11, 1996 <br />DAN MATHEWS <br />1500 feet. Access to the hole LW9-3 will follow existing access that was last used in January 1996 <br />for access to $DHfl6. The total distance of the proposed new access (12') for the hole is about 1700 <br />feet. The drilling location will result in disturbing about 0.23 acre. A total disturbance of 0.7 acre is <br />estimated. <br />Drilling locations are prepared by removing and stockpiling topsoil and vegetation to be used in <br />reclamation. Pits are cut in undisturbed (non-fill) ground to contain cuttings and fluids used while <br />drilling. The pit is pumped out and baddflled and the site recontoured as soon as practical after hole <br />completion. The drill hole sites are disked and seeded with 20 Ib/acre of seed mixture as shown in <br />Table V-14 on page V-49 of the permit. Runoff from the drill sites are controlled as per the permit <br />document language. See Page III-30 of the permit. Also see Sedion IV.J.S. (xvii) Small Area <br />Exemptions exempting degas, sump-dewatering and other drill holes from sediment pond <br />requirements. <br />Page III-29 of the permit has been amended to reflect this minor revision. Three copies of this page <br />and Map 138 are being submitted for your files. A completed revision form is also enclosed. <br />Western Fuels-Utah currently has en adequate cushion in its performance bond submitted to DMG <br />to cover additional bond amount required for the reclamation associated with this minor revision <br />request. Additionally, please note two holes (LW10-1 and LW11-1) previously permitted for drilling <br />under MR97 will not be drilled since the permitted underground area has been sealed. Hole LW9-1 <br />was also permitted previously. This hole was drilled as an emergency hole in a different location. <br />Please credit the bond amounts computed for the previously permitted LW9-1, LW10-1 and LW11-1 <br />holes toward the total bond requirement. Please let me Imow the reclamation bond amount for LW9-1 <br />(relocated), LW9-2 and LW9-3 computed by DMG as well as the credit for the previously permitted <br />but not drilled holes LW9-1, LW10-1 and LW11-1 so we can adjust the bond cushion balance <br />accordingly. <br />Sincerely, ~/ .~~~" <br />Murari P.
