Please indicate in the Reclamation <br />Plan if seeding of stockpiles left in place longer than a year will occur or if other alternate erosion <br />control plans are to be implemented. <br />8) The Reclamation Plan needs to state the varieties of plant species to be used in the seed mix and in <br />what quantities they will be: distributed. Also, in orderto calculate the bond, the manner of <br />planting needs to be specified, i.e. broad casting, drill seed, etc. Please provide this information. <br />9) If no other permits or licen:;es are required such ~s storm water, air quality, land use etc, for this <br />conversion, a statement to that effect must be placed in Exhibit M. <br />]0) Exhibit O should state a complete -ist of all owners and utility easements of the affected land. If <br />they are shown on Exhibit (: Map please reference in this exhibit. <br />11)Per Rule b.4.17, Exhibit Q must show proof of notice to County Commissioners and the Soil <br />Conservation District. Please be advised that without proof of prior service to the Soil <br />Conservation District this exhibit does not meet the rule. The documentation supplied shows proof <br />of mailing not proof of service to the NRCS. If documentation of proof of prior service is not <br />available, a waiver of the public comment period from the Soil Conservation District will be <br />necessary or as per Item 1, the clock and all applicable deadlines begins anew. Further, a statement <br />from the Jim Boyd of NRCS stating that he also is a board member of the Soil Conservation <br />District will need to be submitted to complete this exhibit. <br />12) Exhibit S requires all man-made structures located within 200 feet of the affected land be listed. A <br />notarized agreement between the applicant and the person(s) having interest in these structures that <br />the applicant shall provide compensation for damage to the structures is required. If no agreement <br />is available, the operator must provide appropriate engineering evaluation that demonstrates such <br />structures shall not be damaged by the mining operation. Please provide a list of such structures <br />and the required agreement Or engineering. This list should include buildings, all utilities, fences, <br />irrigation ditches, etc. that are not solely owned by the applicant. <br />13) The property valuation notice supplied is of poor quality. All data is unreadable, so verification of <br />parcel, description and ownership cannot be verified.
