5. Please indicate whether or not additional surface disturbance will occur, such as power pole <br />installation and/or access road construction, in order to install a power supply to the proposed <br />vent fan site. If additional disturbance will occur, these features should be included in the <br />permit area, illustrated on the permit maps, described in the mining and reclamation plans and <br />accounted for in the reclamation cost estimate. Designation of these additional areas may <br />require republishing of the public notice, re-noticing of the landowners, and re-starting the <br />evaluation period for this amendment. <br />6. Please specify if explosives storage will occur on site. If so, please detail the storage facilities <br />and show them on the permit maps. <br />7. Please indicate the measures that will be taken to ensure the waste rock disposal area will not <br />exceed background radiation levels after the site is reclaimed. Include current background <br />radiation levels for this location. <br />8. Please provide specifications of the proposed portal closure. Is a bulkhead all that is proposed? <br />DMG recommends installation of a bulkhead 25 feet inby the portal and then an earthen <br />backfill of the adit outby the bulkhead. Alternatively, if the mine workings are demonstrated to <br />be favored as a hibernaculum for bats, Cotter Corp. may wish to consider installation of a bat <br />grate closure. <br />9. Please provide detailed specifications for the vent closure design and process. <br />10. Please specify the maximum waste rock dump dimensions. DMG recommends a maximum 3:1 <br />dump slope if possible. <br />11.Please specify if mulch or fertilizer will be applied during reclamation and specify application <br />methods and amounts if they are to be applied. DMG recommends application of 1 to 1.5 tons <br />of certified weed free hay mulch be applied and anchored. <br />12. Please provide proof of the 1996 lease renewal and verify that this lease covers the proposed <br />vent hole locations. <br />13.
