The application cannot be approved until the following <br />issues are adequately addressed. Please submit a response to the following issues as soon as possible, <br />but no later than October 20, 2004 so the decision due date set for November 8, 2004 can be met. If you <br />cannot respond by October 20, 2004 please submit a written request for extension of the decision date. <br />If your response, or an extension request, are not received by October 20, 2004 this application may be <br />denied. <br />ADEQUACY ISSUES <br />1.. Please label the permit boundary on all maps, ensure that the entire 14.9 acre permit area is <br />identified on the maps and are identical on all maps (except location mapl, and that all maps <br />are signed and dated by the qualified preparer. <br />2. Please provide details for secondary spill containment for all hazardous materials that will be <br />stored on site. <br />3. Please specify the construction details and dimensions for the proposed portals and vent <br />hotels). <br />4. Please provide a detailed drainage control plan for all proposed areas to be affected, including <br />the portal bench, roads, facilities/staging/parking areas, and stockpile/waste rock disposal <br />areas. The plans must demonstrate the ability of the drainage control features to adequately <br />handle the 10 year 24 hour precipitation event during the operatiori of the mine, and ensure <br />that these features will adequately handle the 100 year event after reclamation. The plan <br />must demonstrate that all mining features will remain stable and that discharge points will not <br />cause off-site sedimentation or erosion. <br />Office of Office of Colorado <br />
