Southfield Mine C-8t-014 Phase 1 Bond Release inspection <br />Dear Mr. Stark and Ms. Brown: <br />RECE6VED <br />NOV 19 2002 <br />Givision of Minerals and Geology <br />~_ m~ Iat u,w say tt18t is was a pleasure meetitrg.both of you atthe.Pllase 1_Bond Release Inspection, <br />trerein referred to as the Inspection. <br />1 am submitting the following oorrcer-ts. ot~ections and request on behalf of Irene Vento Coulter, the <br />owner of 2/3 of the Vento property as they relate to the Phase 1 Redamation, which is the responsibility <br />of F~rergy Fuels Coal Inc. herein, referred to as FOCI: <br />MSHA Report <br />Do you have a copy of the MSIiA report which details the inspection relating to MStfA <br />regulations allowing the shutting down of the fans for up to 90 days before the mine <br />status dtanges to 'Gosed° and subsequent inspection requirements inducting (at a <br />minimum) to walk alt returns and seals (essentialry the perimeter of the open <br />woddngs); spot checks of the areas such as the four sumps; the pumping of waterrf <br />necessary; any fan restart and gas readings at each fan before tt is restarted? <br />• We are requesting a copy of that report and any related follow-up <br />inspection reports done by your agency. <br />2. Portal Area <br />• We are requesting your report on the sealing of the portals inducting <br />the materials and demolition debris that was left in the portals. Our <br />observation and v~eotape shows tires, ileslrs, cinderblocks; trash and <br />blade sludge in addition to scrap metal and concrete filling the portal <br />entrances. <br />Our gravest concern ~ one that trot the state, federal or FOCI representatives <br />addressed but which Dr. Codey, who was also present at the Inspection, brought to <br />everyone's attention and that is the uncapped vent in the Portal Area. Quite honestly, <br />tt is beyond our comprehension that this matter has escaped your observances arxt <br />anaysis considering its viable propensity to spontaneously combust with the resulting <br />effects of underground fires. tt is an oversight and omission which alone gives us <br />grourMs to request that the Bond Release not he granted until furttter investigation <br />and subsequent satisfactory solWons are reached and agreed to by the State of <br />Cobrado, EFCI, Undo Saunders and Irene and Paula Coulter, herein refened to as <br />
