<br />• Limit of disturbance lines are shown outside the limits of mining on the west and north sides of the <br />North Quarry, and on the east of the Central and South Quarries. These minor expansions have been <br />made to allow for access to mine workings and to allow for material stockpiles and berms outside the <br />limits of mining. Berms along the eastern edge of the Central and South Quarries aze included as a <br />measure of reducing visual impacts from the east and southeast. Areas shown between the limits of <br />mining and the limits of disturbance will not be mined and may not be entirely disturbed, but are <br />designed to be used for equipment access and stockpile activities. <br />The type of present vegetation at the quarry is generally characterized by extensive grasslands <br />interspersed with widely scattered, isolated clusters of shrubs. Present vegetation is identical to that <br />described in the previous permit and amendment applications to DMG for the quarry, with the <br />exception of removal of vegetation in mined areas and replacement of vegetation in reclaimed areas <br />since the time of those submittals. Detailed vegetation information is contained in Exhibit ] of <br />Amendment No. 2 for the Morrison Quarry. <br />In compliance with the Rules and Regulations established by the State of Colorado vis 9 vis the Mined <br />Land Reclamation Act, the Division of Minerals and Geology (DMG) and the Mined Land <br />Reclamation Board (Boazd), the following materials outline Cootey's proposed amended mining and <br />reclamation plans for the subject affected land. Specifically, this amendment encompasses submittal <br />or resubmittal of twelve Exhibits required for a Division of Minerals and Geology (DMG) permit <br />application as follows: <br />• Exhibit B -- Index Map <br />• Exhibit C -- Premining Plan and Affected Lands <br />• Exhibit D -- Mining Plan <br />• • Exhibit E -- Reclamation Plan <br />• Exhibit F -- Reclamation Plan Map <br />• Exhibit G -- Water Information <br />• Exhibit L -- Reclamation Costs <br />• Exhibit M -- Other Permits and Licenses <br />• Exhibit P -- Municipalities within Two Miles <br />• Exhibit Q -- Proof of Mailings <br />• Exhibit R -- Proof of Filings <br />• Section 6.5 -- Geotechnical Stability Exhibit <br />All other required Exhibits remain unchanged from Amendment No. 2 and prior submittals, and are <br />not resubmitted herein. In addition, Cooley intends to publish notice in a general circulation local <br />newspaper within ten (]0) days of this submittal and mail copies of the published notice to potentially <br />affected landowners per DMG guidance. <br />This permit application has been prepazed by Haley & Aldrich, Inc, under the direction of Tracy ]. <br />Lyman, P.E., a registered Professional Engineer in the State of Colorado specializing in civil and <br />geotechnical engineering. The mining plan and slope stability analyses were developed by Mr. Kent <br />A. Pease, P.E., also a Colorado Professional Engineer. The revisions to the reclamation plan and the <br />remainder of the submittal were completed by Mr. Alan L.
