Phase Nc Summary No. 13 Page 2 043-2170 <br />screen plant by the secondary crusher <br />• Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase <br />1Vc OSA. Geomembrane was installed in the Phase PJc OSA on the south facing slope of Raven Hill <br />downwazd to the Phase IVb/IVc tie-in. Poly-Flex deployed 80-mil single-sided textured LLDPE <br />geomembrane over an azea of approximately 231,085 squaze feet in the Phase IVc OSA during this <br />reporting period. Geomembrane panels C-485 through C-569 were deployed, seamed, and tested by <br />Poly-Flex. Destructive fusion and extrusion seam samples DFc-174 through DFc-200 and DXc-18 <br />through DXc-21 were mazked and removed. Destructive fusion and extrusion seam peel and shear <br />testing on samples DFc-167 through DFc-187 and DXc-17 through DXc-20 were performed and passed <br />per project specification. <br />Ames continued hauling, placement and grading of DCF material into the Phase IVc OSA in the <br />western and central portion of the VLF, Raven Hill azea. Ames placed DCF material and High Volume <br />Solution Collection (HVSC) piping per the project plans onto approved geomembrane panels. <br />• <br />I • <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Field Monitorine and Testin¢ Activities <br />Golder observed construction activities associated with Phase Nc earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase 1Vc OSA for <br />subgrade preparation, thickness, compaction, moisture-content, and materials used. The Golder CQA <br />earthworks monitor performed twenty nine nuclear moisture-density tests (SLNc-142 through SLNc- <br />171) and performed six nucleaz moisture-density retests on SLF that required reworking (SLNc-154a, <br />SLNc-156x, SLNc-158a, SLNc-162x, SLNc-167a, and SLNc-169a). Field test results aze presented in <br />Attachment B. <br />Golder observed geomembrane deployment, documenting ambient temperature, panel dimensions, <br />' thickness, seaming, tie-in overlap, and seam testing-(trial seams, pressure testing). Geomembrane <br />installed in the Phase IVc OSA met the project specification. <br />Golder visually observed the placement of DCF in the Phase IVc OSA area. Golder CQA monitors <br />(one monitor per dozer when dozers aze greater than 50 feet apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillar <br />Mode1740 articulated trucks). <br />Golder observed placement of HVSC piping in the Phase IVc OSA, verifying installation and layout <br />per the project plans. <br />rnss~rvc u.w~,.~i-w~c nooa.aa GOIder ASSOCIateS lnC. <br />
