Phase IVc Summary No. 7 Page 3 043-2170 <br />• Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers aze greater than 50 feet apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillaz <br />Mode1740 articulated trucks). <br />Golder observed placement of HVSC piping in the Phase IVc OSA, verifying installation and layout <br />per the project plans. <br />Golder visually observed removal of DCF on June 25 from the Phase IVc OSA area to allow for <br />inspection of the Phase IVc geomembrane due to equipment damage. Golder marked the defect <br />damage (29K) caused to the geomembrane, then repaired, non-destructively tested, and passed the <br />defect during this reporting period. <br />Laboratory Tests <br />Golder obtained one QC sample of bentonite-amended SLF (SLF-QC-14). Laboratory index testing <br />was conducted on the sample of bentonite-amended SLF at the Golder Soils Laboratory in <br />Lakewood, Colorado, and the CC&V field trailer to verify suitability of materials prior to use in the <br />field. A summary of QC laboratory test results for SLF materials aze available at the Golder field <br />trailer. <br />Golder obtained no QA samples of during this reporting period. <br />• Meetines <br />CC&V held a Weekly Contractors' meeting on June 22, 2004. Items discussed included Safety, <br />Environmental, Schedule, QC documentation, and CQA.
