'Phase Nb Weekty Summary No. 27 Page 3 003-2203 <br />Earthworks monitor performed nine nuclear moisture-density tests (SLNb-191 through SLNb-199) <br />and performed five nuclear moisture-density retests on SLF that required reworking (SLNb-188a, - <br />189b, -191 a, -193a, and -196a) located within the Phase IVb OSA. The SLF met the requirements of <br />the specifications. Field test results are presented in Attachment B. <br />Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA, <br />verifying construction and layout per the project plans. <br />Golder observed and documented excavation and remediation activities associated with the Phase IV <br />underground working construction. <br />Golder visually observed placement and grading of DCF in the Phase IVb OSA. Golder CQA <br />monitors (one monitor per dozer when dozers aze greater than 50 ft apart) observed DCF placement <br />and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations, <br />5 feet for 773 equipment, and 10 feet for haul road construction and equipment. <br />Golder visually observed removal of DCF from the Phase 1Vb / Phase I /Phase II tie-in azea, at the <br />old LOB pad access haul road. Golder mazked damage caused to the geomembrane for subsequent <br />repair during Phase IVb geomembrane tie-in construction. <br />Golder observed anchor trench excavation in the Phase Nb OSA for proper location and dimension <br />(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. Golder also provided part-time observation for anchor trench backfilling, verifying <br />construction par project plans. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in the Phase IVb OSA met the project specifications. <br />Laboratory Tests <br />Golder obtained QA samples of Drain Cover Fill (DCFb-12), and Soil Liner Fill (SLFb-19). <br />Laboratory index testing was conducted on samples of Drain Cover Fill (DCFb-12), and Soil Liner <br />Fill (SLFb-19) to verify suitability of materials used. Laboratory test results are presented in <br />Attachment B. <br />Meetin¢s <br />CC&V held a Weekly Contractors meeting on July 22, 2003. Items discussed included Safety, <br />Environmental, Schedule, and CQA.
