Long <br />RE: File No. M-1989-084, Oak Creek Sand and Gravel Pit, Conversion Application Diva:.on Director <br />The following; items must be addressed by the applicant, in order to satisfy applicable requirements of C.R.S. <br />34-32.5 101 et seq. and the Construction Materials Rules and Regulations of the Colorado Mined Land <br />Reclamation 13oard. <br />[l'a ~f l~eneral Regrtirements <br />~5~ ~, <br />4 1. Rule 6.2.1 requires that maps, except the index map, be signed by a registered land surveyor, <br />profe:;sional engineer, or other qualified person. Please submit two copies of the pre-mining/mining <br />plan :tnd reclamation plan maps with an appropriate signature. Please do this after any changes you <br />make to the maps, responding to the remainder of this adequacy review. Please also include the date [he <br />maps are prepazed. <br />Exhibit C-Prc-Mining and Mining Plan Map(s) of Affected Lands <br />oN" 1. Rule 5.4.3 requires that this map legibly portray the total azea to be involved in the operation affected <br />land. You show an Affected Land boundary as a solid red line. The Division assumes that this line is <br />also the permit boundary. If it is, please add that information to the map. If there are areas within the <br />proposed permit boundary that will not be affected by the mining operation as indicated by the <br />reclac anon plan map, then it will be necessary for you to add a separate affected land boundary to your <br />map showing the proposed extent of the land to be affected (disturbed). <br />p~A 2. Rule ~5.4.3(g) requires that the map show the name of the owner of permanent man-made structures. <br />Please show the name of the owner of the powerline. <br />X11 3. Show on this map the present extent of affected land. <br />berm?
