STORMWATER -INDUSTRIAL ACTIVITY
FIELD AUDIT /INSPECTION FORM (PAGE 2 OF 2,
Date:
January 7, 1997 Inspectors: Cert No.
David W. Knope COG - 500912
Facility Name: The Aggregate Source, lnc.
D. Waste materialslstorage exposed to stormwater? Yes No ~
Observations: No major waste areas were observed. Few tailings are present as the majority of the material
that is quarried is sold as aggregate and rip rap.
E. Vehicle maintenance or fueling; storage of fuel? Yes ~ No
Observations: The above ground fuel storage tanks did not have a spill comainment structure.
F. Evidence of other exposed industrial activities? Yes ~ No
Observations: Vehicle maintenance is to be performed inside of a recently constructed building.
G. Evidence of non-stormwater discharges? Yes No ~
Observations: Aggregate washing is no longer perfomred at the quarry. Due to the lack of process water at the
site, it is believed that the stormwater permit is sufftciem and a t:DPES permit is not required.
H. Best Management Practices, installation, maintenance, effectiveness; additional comments:
• In order to control the starmwater runoff, it is recommended that water diversion structures (ditches, culverts,
rolling dips, etc.J be constructed to channel the water into the retention ponds.
• A general housecleaning schedule should be developed to pick up discarded material (oil contai~hers, plastic
wrapping, snap metal, etc. J that was scattered around the site.
• The spillway jot Pond q3 had been constructed. The spillway for Pond N2 was roughed in alon,„side the road.
The spillway jot Porul NI was not well defined. /t is recommended that the spillways jot Ponds til and q2 be
worked on to provide a more defined structure. ALro, the retention ponds should be worked on to ensure that
they are constructed to general specifications (i.e. 3 to ]side slopes on embankments) and have ~7dequate
volume to contain the anticipated flow volume.
• Any erosion below Pond N3 would impact Tunnel Drive due to lack ojerosion control below this point.

c~~a loco
