Klco
Title/Responsibilities: Consulting Geologist
Nature of the Industrial Activity:
Rock Quarry
SWMP available on site? Yes ~ No Photographs Taken? Yes ~ No
SWMP requested previously and reviewed by inspector? Yes No ~
Receiving Water: Hydraulic Cana! & Arkansas River

Further Action Required by Division: 1. Follow the progress made by the mine to ensure that stornwater runoff
from the site is controlled. 2. Address the stornwater management plan for the proposed expansion of the quarry.
Further Action Required by Permittee: 1. Submit by March 10, 1997, to the Division, a written not cation that
aggregnre washing is no longer performed at the quarry. T1tis will be used to modem the stornwater management
plan. 2. Spill containment shall 6e constructed around the above ground storage tanks. 3. !n order to minimize the
amount of runoff leaving the site along the quarry road diversion structures should 6e constructed to channel the
water into the retention ponds. Also, practices should be implemented on the access road below Po.ad p3 to minimize
the erosion impact from this stretch of road on Tunnel Drive.
Date Required: March 10, 1997
Signature of Representative:
A. Evidence of stornwater discharge from facility? Yes ~ N~>
Observations: Several areas of water erasion were observed along the quarry road. Due to the lack of adequate
diversion structures, all of the runoff may not be channeled into the retention structures. Any erosion
below Pond N3 would impact Tunnel Drive as there are no control structures below this point.
B. Materials stored outside? Yes ~ No
Observations: The finished product is stored outside near the crushing/sneening operation. This practice should
not appreciably impact the runoff from Jhe site.
C. Shipping and Receiving Areas exposed to stotmwaterl Yes ~ No
Observations: The rip rap loading area is exposed ro stornwater. This practice should not appreciably impact
the runoff from the area due to the inert nature of the product and the storage area being locate.i up gradient
from the retention pond.
