The <br />operator paid the $700.00 on August 26, 1993. <br />Notice of Violation C-93-052 <br />Issued April 12, 1993 for failure to properly install sections of approved upland <br />diversions. The NOV was terminated on December 27, 1993, following <br />veriScation of abatement completion. At the assessment conference, the NOV <br />and proposed fine were upheld. The operator paid the fine of $1,700.00 on <br />August 26, 1993. <br />Notice of Violation C-93-084 <br />Issued June 4, 1993 for the east facing slope of the refuse pile being steeper than <br />2.OH:1.OV. Rule 4.10.4(4), which addresses slope angles for terraced slopes, was <br />cited. Subsequent to the NOV issuance, the operator demonstrated to the <br />Division that the slopes of the refuse pile that were the subject of the NOV were <br />in fact not terraced, and as such 4.10.4(4) did not apply. The NOV was <br />consequently vacated by the Division Director on December 7, 1993. <br />Notice of Violation C-93-087 <br />Issued June 10, 1993 for failure to pass surface drainage from disturbed areas <br />through a sediment pond, a series of sediment ponds, or a treatment facility <br />before leaving the permit area. The NOV abatement deadline was extended <br />while the resulting TR and field work were completed. The NOV was terminated <br />December 27, 1993. The operator paid a fine of $1,200.00 on February 18, 1994. <br />Cessation Order C-93-088 <br />Issued June 10, 1993 for failure to abate NOV C-93-050. The mine identification <br />signs at the Santa Fe 1 and 2 I.oadouts lacked information required by 4.02.2(2), <br />as previously addressed by NOV C-93-050. Following an assessment conference <br />held to discuss CO C-93-088, it was vacated by the Division Director on August <br />25, 1993. <br />Notice of Violation C-93-150 <br />Issued December 3, 1993 for failure to have a qualified registered PE, or <br />individual under direction of same, conduct quarterly inspections of refuse pile; <br />failure to have a qualified registered PE, or individual under direction of same, <br />
