The application cannot be approved until the following issues are <br />adequately addressed. Please submit a response to the following issues as soon as possible, but no <br />later than November 17, 2003 so the decision due date set for December 1, 2003 can be met. If you <br />cannot respond by November 17, 2003 please submit a written request for extension of the decision <br />date. If your response, or an extension request, are not received by November 17, 2003 this <br />application may be denied. <br />ADEQUACY ISSUES <br />If the pre-mining and mining plan maps submitted with this application are accurate, then it <br />appears the mining plan, specifically the north buffer zone for the AM-01 area, has once <br />again been disregarded and may be jeopardizing the stability of the fence along that <br />boundary. <br />2. Please submit proof of publication and notices as required by Rule 1.6.2(11(d) and (el• <br />3. The sign posting affidavit submitted with the application has not been filled out. Please -- <br />submit acompleted and executed affidavit of sign posting.
