Portions of the Hubbazd Creek road and the Tenor Creek road are mentioned in Section <br />2.05.6(6)(a)(ii)(A) on revised page 2.05-103. However, only portions of the Hubbard <br />Creek road are described in Section 2.05.6(6)(b)(i)(C) on revised page 2.05-105. Please <br />revise page 2.05-105 to include a description of the projected effects from subsidence on <br />portions of the Terror Creek road. <br />11. In order to aid in the review process and under Rule 2.05.6(6)(a)(ii)(B), please add <br />depictions to Map 27 for those portions of the Hubbazd Creek road and the Terror Creek <br />road that are within the permit and adjacent areas. <br />12. On revised page 2.05-107, under Section 2.05.6(6)(f)(i), it is stated that Map 27 depicts <br />the depth of cover. The depth of cover numbers are present but the contours are bazely <br />visible. Please revise Map 27 to show the depth of cover contours clearly. <br />13. On revised page 2.05-107, there is an entry for Rule 2.05.6(6)(c)(i)(t). However, this rule <br />applies to the subsidence monitoring program, which is relevant only if there is a <br />determination of no material damage or diminution of reasonably foreseeable use. In <br />order to avoid any confusion, please omit this rule reference and move the information <br />concerning the cessation of the subsidence monitoring program to the text pertaining to <br />Rule 2.05.6(6)(f)(v)(B) where the subsidence monitoring program is described. <br />14. On revised page 2.05-107, there are entries for Rules 2.05.6(6)(e) and (e)(F)(iv). <br />However, Rule 2.05.6(6)(e) applies to the subsidence survey, which BRL has elected not <br />to use in favor of the subsidence control plan. In order to avoid any confusion, please <br />omit these two rule references and move the text information to Section <br />2.05.6(6)(b)(i)(B). <br />I5. The seismic monitoring program is mentioned in Section 2.05.6(6)(I)(v)(B) on revised <br />page 2.05-117, and is described in Exhibit 18 of Volume 3A. However, certain specifics <br />aze missing from the seismic monitoring program description. Please revise Exhibit 18 of <br />Volume 3A to include specific details on the start, frequency, duration and termination of <br />the seismic monitoring program and the reporting format and frequency of submittal of <br />the seismic data and interpretation. Also, on revised page 2.05-117, please add a general <br />overview discussion of the seismic monitoring progmm and report requirements. <br />16. The subsidence monitoring program is described beginning on revised page 2.05-115 in <br />Section 2.05.6(6)(f)(v)(B). However, not all of the subsidence monitoring is required by <br />the Division. A portion of the subsidence monitoring is required by the Division but a <br />portion appazently is required by the USDA-Forest Service. Some portions may have <br />
