On March 19, 2007, the Division of Reclamation, Mining and Safety received a Transfer of Permit and Succession of
Operators Application, Performance Warranty, and a $100.00 fee for the Lily Mine, Permit No. M-1987-028. The
Division is returning these documents for the following:

The Division requires the Current Permittee company name listed throughout this document match the company
name listed with the Colorado Secretary of State's office (i.e. Colorado Marble, L.L.C.). Please change the
company name listed on pages 2, 4 and 5. Please find Secretary of State documentation enclosed for your
reference.
The Division requires the Successor Operator company be listed with the Colorado Secretary of State's office.
Performance Warranty:
The Division requires the Successor Operator company be Listed with the Colorado Secretary of State's office.
The company name must be listed consistently throughout this document. Please include a comma within the
company name listing in the signature and notary sections on page 3.
